Have a look at our novel Direct Volume Rendering solution AVIS (Adaptive Volumetric Illumination Sampling). It approximates realistic lighting for volumetric data (e.g., CT data) in real-time. However, unlike other approaches, AVIS does not introduce additional noise and allows for unrestricted interaction with the camera, clipping planes, and the transfer function.
